Blue Cross of Northeastern Pennsylvania and Guthrie Medical Group have implemented a new Patient Centered Medical Home pilot program.

The program is designed as a collaborative effort between BCNEPA and Guthrie to improve patient care allowing for: the improvement of health and delivery of services of Guthrie patients while reducing the cost.

Thomas Novinger, senior medical director at BCNEPA said in an emailed statement, “BCNEPA’s staff of Blue Health Solutions Care Managers will have access to the same tools and reports and Guthrie Medical Group staff, enabling them to work together to manage the patient’s care needs.”

“This pilot has played a role in the transformation of Guthrie’s medical practices into patient centered medical homes,” Frederick Bloom, president of Guthrie Medical Group, said in a press release. “We are pleased that we have been able to work with BCNEPA on part of this journey to benefit of our patients.”

Guthrie Medial Group has 25 locations in Pennsylvania, with the closest being Tunkhannock and Towanda.
