The following are recent Luzerne County restaurant inspection reports from the Pennsylvania Department of Agriculture. According the agency: “any inspection is a ‘snapshot’ of the day and time of the inspection. An inspection conducted on any given day may not be representative of the overall, long-term cleanliness of an establishment. Also, at the time of the inspection violations are recorded but are often corrected on site prior to the inspector leaving the establishment.” The information is taken from the inspection database at www.eatsafepa.com. Postal addresses used here are as listed on the state’s website, and may not correspond to the municipalities in which facilities are physically located.

Brothers Shim, 9701 Bear Creek Blvd., Bear Creek Township. June 16. Regular inspection; in compliance. Violations: Observed a mineral buildup along the drip edge of the plate inside the ice machine. Person in charge cleaned ice machine. Observed a mineral build up in the ware wash sinks in the bar area.

Camp Kresge, 382 Senator Raphael Musto Dr., White Haven, June 16. Regular inspection; out of compliance. Violations: Person(s) in charge are not monitoring facility operation for compliance with food code requirements. Food employees observed in food prep area, not wearing proper hair restraints, such as nets or hats. Two 1/2 gallons of milk stored in walk-in cooler are showing expired sell by date. Milk was voluntarily disposed. Cleaned bowls and service plates are displayed with food contact surfaces up, exposing food contact surfaces to contamination in dining room. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration in chemical sanitizing dish machine final rinse. Hoses and sprayer connected to hose bib by curb sink do not have backflow preventers installed between hoses and hose bibs. Cob webbing accumulations are noted in dining area and above and below food prep areas in kitchen. Cleaning should occur prior to occupation and be monitored and cleaned as needed during camp use. Semi-soft rodent droppings observed in dry store rooms and in small wall cabinets in dining room. Discussed control and monitoring measures.

Hickory Run Plaza Food Mark, 2250 Rte. 534, White Haven, June 16. Regular inspection; in compliance.

Grotto Pizza, 337 Wyoming Valley Mall, Wilkes-Barre, June 19. Regular inspection; in compliance. Violations: Multiple food ingredient storage containers, in the kitchen area, not labeled with the common name of the food. Containers were labeled. Observed a buildup of black residue around the hinges on the pasta cooler. Person in charge cleaned area. Observed a buildup of white mold-like substance in the bottom of the chest beer cooler in the bar area. Employee cleaned area. Observed the plastic coating deteriorating on the metal shelves in multiple refrigeration units. Deteriorating rubber door gaskets observed on the bain marie in the cooking area. Observed a buildup of black residue on the surface above the nozzles on the beverage machine. Observed a buildup of dust on the fan guard in the bain marie in the kitchen. Observed a buildup residue on the ceiling of the sauce bain marie. Employee observed emptying ice into the hand-wash sink in the wait staff area.

Mr. Moo Cow MFF3, 1280 Rte. 315, Wilkes Barre, June 19. Regular inspection; in compliance. Violation: Sewage/wastewater from the food facility is not being disposed of properly to an approved sewage system.

Northwest Area High School, 243 Thorne Hill Rd., Shickshinny, June 19. Regular inspection; in compliance.

Curry Donuts, 733 Wyoming Ave., Kingston, June 19. Follow-up inspection; in compliance. Violation: Roof is leaking water in left corner of the mixing room, and is in need of repair. The ceiling has been repaired however it is raining today and there is now water dripping from that area.

Swift Concessions – Northeast Fair, Pittston, June 20. Regular inspection; in compliance. Violations: The food facility does not have the original certificate for the certified food employee posted in public view. An email was sent to the owner to forward a copy of certificate to this inspector. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the under counter refrigerator. A thermometer was placed in the unit during this inspection. Temperature was observed at 37 degrees F. No sign or poster posted at the hand-wash sink to remind food employees to wash their hands. A sign was posted during this inspection.

Violations: Commercially processed refrigerated, ready-to-eat, time/temperature control for safety food, located in the refrigeration units, and held more than 24 hours, is not being marked with the date it was opened. Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Employee in food prep area observed with tobacco chew in mouth. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Observed wet wiping cloths in kitchen area, not being stored in sanitizer solution. Food containers stored on floor in walk-in cooler. Meatballs and sauce in hot holding are 70 degrees F product center. Employee states food was just put in and not reheated. Food removed for reheat to 165 degrees F. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Observed deeply scored cutting boards not resurfaced or discarded as required. Slicer has buildup of old dried food residuals on all food contact surfaces. Slicer cleaned during inspection. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil and food splash. No chemical sanitizer is being used as third step in manual ware-wash process as observed while watching employee cleaning dishes. Rear food prep door is open and unscreened.

Violations: Frozen hamburgers observed not maintained in a frozen state in the meat freezer by the cooking area. Foods were discarded and unit will be serviced prior to use. Observed the coating deteriorating on the metal shelves in the walk-in cooler. Observed a buildup of mold-like residue on the back surface of the ice chute in the self-serve area. Ware-wash sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Hose was cut during inspection.

Violations: Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Two packs of cigarettes and 1 open box of screws and 1 open box of nails are stored above chest freezers whose lids are used for food prep. Food facility unable to provide documentation for procedures, monitoring of critical control points, and corrective actions in the HACCP plan submitted to and approved by the Department. No daily log for PH of acidified rice for use in /Sushi items. Last entry was November 2016. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Thermometers for ensuring proper temperatures of food are not available or readily accessible. No temperature tapes (160 degrees F) or temperature holding thermometer is available to monitor final rinse plate surface temperature in high temperature dish washer. Two hoses connected to hose bibs in food prep/food storage areas do not have backflow preventers installed between hoses and hose bibs. Rear food prep screen doors are not tight fitting to framework.

Leaf & Ladle, 59 S. Mountain Blvd., Mountain Top, June 21. Regular inspection; in compliance. Violations: Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Food container stored on floor in walk in freezer. Residual date sticker material observed on stacked, cleaned food pans.

Lyons Den, 34 Orchard St., Glen Lyon, June 21. Regular inspection; in compliance. Violations: Ice scoop lying with food contact surface directly on top on the ice machine located in the basement. The ice scoop was cleaned and sanitized during this inspection. Chicken and cheese was held at 47-48 degrees F, in the two-door reach-in Advan-EDGE refrigeration unit, rather than 41 degrees F or below as required. The owner called and arranged for repairman to look at the refrigeration unit. All TCS foods (chicken and cheese) were removed and discarding during this inspection. The Person in charge has agreed not to utilize this refrigerator to store any TCS foods until it is repaired and holding temperature at 41 degrees F or below. Owner has called a repairman to come look at the refrigeration unit.

Modern Lanes, 1948 Wyoming Ave., Exeter, June 21. Regular inspection; in compliance. Violations: Observed a buildup of rust on the rails in the refrigerator and on the bottom of the chest beer cooler. Observed a buildup of mold like residue and rust on the deflector plate in the ice machine. Machine was shut off and will be cleaned and sanitized prior to use. Observed a buildup of syrup residue on surface above nozzles on the beverage machine. Observed clean glassware in the bar area, stored uncovered or not inverted.

Observed a buildup of food residue on the exterior of the knife holder in the deli area. Knife holder was cleaned during inspection. Observed a buildup of residue on the honey comb and around the misting nozzles in the produce area. Observed Styrofoam meat trays stored directly on the floor in the meat room storage area. Trays were moved off the floor during inspection.

Salem Twp., Volunteer Fire Co., #1, TFS 3. Fire Company Lane, Beach Haven, June 21. Regular inspection; in compliance.

Smirks, 1310 Main St., Duryea, June 21. Regular inspection; in compliance. Violations: Observed raw beef stored alongside bagged ice in the freezer. Beef was moved. Observed knives being stored in a wooden block. Knives were removed and block was discarded. Observed peeling coating and rust forming on shelves in the refrigerator. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed a buildup of residue in the ware-wash sinks in the kitchen and bar areas. Observed a buildup of old food residue on the interior of the microwave. Person in charge cleaned microwave.
