Luzerne County restaurant inspections June 22-29: Rodent droppings, mold build-up lead to out of compliance

The following are recent Luzerne County restaurant inspection reports from the Pennsylvania Department of Agriculture. According the agency: “any inspection is a ‘snapshot’ of the day and time of the inspection. An inspection conducted on any given day may not be representative of the overall, long-term cleanliness of an establishment. Also, at the time of the inspection violations are recorded but are often corrected on site prior to the inspector leaving the establishment.” The information is taken from the inspection database at www.eatsafepa.com. Postal addresses used here are as listed on the state’s website, and may not correspond to the municipalities in which facilities are physically located.

Aldi #97, 481 Pierce St., Kingston, June 22. Regular inspection; in compliance. Violations: Women’s toilet room is not provided with a covered waste receptacle for sanitary napkins. There is an area approx. 6” X 4’ where the floor tile is missing and in the walk-in cooler (meat area); the area is not smooth and easily cleanable.

Brenda Michael’s Ice Cream- MFF3, 7 Krushka Rd., Shickshinny, June 22. Regular inspection; in compliance.

Subway #11118, 20 S. Wyoming Ave., Edwardsville, June 22. Regular inspection; in compliance. Violation: Observed pickles in basin of hand-wash sink indicating use other than hand washing. The sink was cleaned during this inspection.

Country Kettle Corn - TFF1, Northeast Fair, Pittston, June 23. Regular inspection; in compliance.

Fratelli’s Italian Pizza, 300 Main St., Conyngham, June 23. Follow-up inspection; in compliance. Violations: No chemical test strips available to monitor chemical sanitizing dish machine. Floor area underneath and behind cook line has buildup of grime/spilled food residuals.

Pamela’s Ice Cream, 7990 Bear Creek Blvd., Bear Creek, June 23. Regular inspection; in compliance.

Brenda Michael’s French Fries, #1 – MFF3, 7 Krushka Rd., Shickshinny, June 26. Regular inspection; in compliance. Violations: Food employees observed in the food preparation area, not wearing proper hair restraints. Corrected. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

Dino’s Italian Restaurant, Inc. 28 Wyoming Valley Mall, Wilkes-Barre, June 26. Regular inspection; in compliance. Violations: Food employees observed in prep area, not wearing proper hair restraints, such as nets or hats. Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Discussed procedure and provided guidance. Loose rubber door gaskets observed on the refrigeration unit in the kitchen area. Observed a buildup of rust on shelves in the walk-in cooler. Observed deeply scored cutting boards not resurfaced or discarded as required. Observed a buildup of old food residue along the back edge and in the cracks of the bain marie in the back area.

Five Guys Burgers and Fries #1715, 280 Mundy St., Wilkes-Barre, June 26. Regular inspection; in compliance. Violations: The water temperature of the wash compartment of the manual ware washing equipment was 97 degrees F, less than 110 degrees F, as required. Food facility hot water heater is not producing enough hot water to supply ware-wash and hand-wash sinks at the time of this inspection. Ware-wash sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Hose was cut during inspection.

Flamingo Diner, Route 11 West Nanticoke, June 26. Regular inspection; in compliance. Violations: Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Person in charge will register for course and provide copy of certificate to inspector when received. Food employees observed in prep area not wearing proper hair restraints, such as nets or hats. Beef, soup and cut melon held at 45-56 degrees F, in the back area, rather than 41 degrees F or below as required. Foods were discarded. Observed 2-door refrigeration unit in the back area with an ambient temperature of 57 degrees F and not 41 degrees F or less as required. TCS foods were discarded and unit will serviced prior to use. Interior surface of both microwaves are cracked and rusting and need to be replaced. Observed metal shelves rusting in the 2 door refrigeration unit in the back area. Observed a buildup of old food residue on the meat slicer. Slicer was cleaned during inspection. Ceiling tiles damaged and missing in the back area, and need replaced.

The Pines Eatery & Spirits, 8 W. Broad St., Ste. 50, Hazleton, June 27. Regular inspection; in compliance. Violation: Observed deeply scored cutting boards not resurfaced or discarded as required.

Turkey Hill #249, 7 Airport Rd., Hazle Township, June 27. Complaint inspection; in compliance.

Twentieth Street Luncheonette, 922 N. Alter St., Hazleton, June 27. Follow-up inspection; in compliance. Violation: Observed equipment, in food prep area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Some cleaning is noted, additional cleaning is needed.

Bingo’s Legendary Hoagies, 503 Slocum St., Swoyersville, June 27. Regular inspection; in compliance.

Brother Bruno Restaurant, 984 N. Sherman Ct., Hazleton, June 27. Regular inspection; in compliance. Violations: Food containers stored on floor in walk-in cooler. Florescent light tubes over pass-thru are not shielded. Fan guards in walk-in cooler have buildup of static dust.

Camp Orchard Hill, Inc., 640 Orange Rd., Dallas, June 27. Regular inspection; in compliance. Violation: The post can opener blade, a food contact surface, was observed to have food residue and was not clean to sight and touch. The post can opener blade was cleaned and sanitized during this inspection.

Carmen’s Bakery & Deli, Inc., 37 E. Broad St., Hazleton, June 27. Regular inspection; in compliance. Violations: Wire shelving in under counter cooler in deli has deteriorated surface coating. Florescent light tube in bakery display case is not shielded. Rear store room door is exhibiting an approx. 1” gap in lower left hand corner.

Crabby’s Seafood Grill, 3159 Lower Demunds Rd., Dallas, June 27. Complaint inspection; in compliance. Violation: Clams located in the reach-in refrigeration unit did not have identification tags attached to the container. The most recent tag was placed in the newly opened container of clams.

Domino’s Pizza, 200 Kennedy Blvd., Pittston, June 27. Regular inspection; in compliance. Violations: Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Person in charge directed to provide copy of update certificate to inspector when received. Food (chicken, sausage, beef) was held at 45-47 degrees F, in the bain marie, rather than 41 degrees F or below as required. Foods held less than 4 hours were placed in the walk-in cooler. Unit will be serviced prior to use for TCS foods. Bain Marie observed with an ambient temperature of 50 degrees F. Foods were removed and unit will be serviced prior to storage of TCS foods. Observed a buildup of old food residue along the edges, in the corners and back vents in the small bain marie. Observed a buildup of residue on the hand-wash sink in the prep area.

Violation: The post can opener blade, a food contact surface, was observed to have food residue and was not clean to sight and touch. The can opener was cleaned and sanitized during this inspection.

Holiday Inn Express, 30 Concorde Dr., Dupont, June 27. Regular inspection; in compliance. Violations: Food employees observed in prep area, not wearing proper hair restraints, such as nets or hats. Observed coating peeling and rust forming on the shelves in the refrigeration unit in the kitchen. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration

Kmart #7142, 1874 N. Township Rd., Pittston, June 27. Regular inspection; in compliance.

Nino’s Pizza and Restaurant, 162 Rte. 239, Shickshinny, June 28. Change of owner inspection; in compliance. Violations: Working spray bottle of a blue liquid in service area, used for storing a sanitizer taken from bulk supplies, was not marked with the common name of the chemical. The bottle was labeled during this inspection. The deflector plate inside of the ice machine, a food contact surface, was observed to have a tan slime and was not clean to sight and touch. The deflector plate was cleaned and sanitized during this inspection.

Old Navy #6473, 437 Arena Hub Plaza, Wilkes-Barre, June 28. Regular inspection; in compliance.

Sheetz #344, 1233 Rte. 315 Blvd., Plains Township, June 28. Regular inspection; in compliance. Violations: Observed flaking foam gasket above the ice bin in the espresso prep area. Observed a buildup of pink residue along the edge of the drop chute in the ice machine. Person in charge cleaned area during inspection. Observed a buildup of residue on the back of the ice chute on the self-serve beverage machine. Employee cleaned area. Observed a buildup of syrup residue on surface behind nozzle on self-serve beverage machine. Employee cleaned area. Ware wash sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Hose was cut during inspection. Observed 5 small flies under the sink in the espresso prep area.

Camp Shehaqua/ Eastern PA 4H, RR1 Hickory Run St., Park, White Haven, June 28. Change of owner inspection; in compliance.

Efuel, HC 1 - 9B Rte. 80 & Rte. 534 White Haven, June 28. Follow-up inspection: out of compliance. Violations: Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Chicken was cooked to an unknown temperature and not 165 degrees F for 15 seconds as required. Cook states he did not take temperature prior to placement in hot hold. Product voluntarily disposed. Assorted temperature controlled for safety foods in hot holding are in the temperature range between 90 degrees F and 96 degrees F product center. Owner states establishment only cooks foods to 80-90% prior to placement in hot cabinet. All items voluntarily disposed. Assorted temperature controlled for safety hot food was held at 90 degrees F to 93 degrees F, in the hot hold cabinet, rather than 135 degrees F or above as required. All items voluntarily disposed. Assorted temperature controlled for safety food was held at 50 degrees F to 53 degrees F, in the refrigerated sandwich unit and in the bain marie, rather than 41 degrees F or below as required. All items voluntarily disposed. Bain marie in use is approx. 50 degrees F ambient air temperature inside. Cold sandwich unit in use is approx. 52 degrees F ambient air temperature inside. Both units were emptied of temperature controlled for safety foods and may not be used until able to maintain 41 degrees F product temperature. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Observed deeply scored cutting boards not resurfaced or discarded as required.

Fujiyama Restaurant, 47 N. Main St., Pittston, June 29. Regular inspection; out of compliance. Violations: The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Multiple food items in the walk in cooler stored open with no covering. Foods were covered during inspection. Multiple food ingredient containers not labeled with the name of the contents. Food utensils in kitchen area observed stored in a container of water which is not maintained at 135 degrees F. Observed in-use knives and cleavers stored between table edges or between tables, an area not easily cleanable & sanitized. Observed wet wiping cloths in sushi prep area, not being stored in sanitizer solution. Observed a cracked lid on a bulk ingredient storage container. Temperature measuring device for ensuring proper temperature of equipment is not available in the sushi refrigerator in the front area. Observed shelf alongside ware-wash to be rusting and no longer easily cleanable. Observed the interior lid of the rice cooker to be cracked and in need of replacement. Observed deeply scored cutting boards not resurfaced or discarded as required. Chlorine chemical sanitizer at the mechanical dishwasher was empty. Facility will use 3-bay sinks to sanitize until product is replaced. Observed a buildup of black mold like residue on the interior of the ice machine. Machine was shut off and will be cleaned and sanitized prior to use. Observed a buildup of food residues on the shelves in the walk in cooler. Observed cardboard which is extremely soiled lining the shelves in the walk in cooler. Observed a buildup of dust on the fan guard in the walk in cooler. Observed a buildup of flour spillage on the shelves in the bulk storage area. Observed a buildup of grease on the sides and under the cooking equipment. Observed a buildup of old food residue on the exterior of the rice cooker and the Bain Marie. Observed a buildup of residue on the ware wash drying rack. Observed a buildup of residues on the exterior of the bulk ingredient containers. Observed old food residue and mold-like substance on the plastic crates in the walk-in cooler. Observed soiled cardboard covering the floor in the walk-in cooler. No paper towels available at hand-wash sinks. Paper towels were provided. Ceiling tiles missing in the back storage area, and need replaced. Kitchen area of the food facility is dirty and grime and in need of cleaning. Observed 50 rodent-like droppings under equipment in the kitchen area.

Martini’s Café, 106 Susquehanna Ave., Shickshinny, June 28. Regular inspection; in compliance. Violations: Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing glass washer was <10 ppm and not 50-100 ppm as required. A new bottle of sanitizer was applied and the residual sanitizer tested at 50 ppm. The deflector plate and left side of the ice bin inside of the ice machine, a food contact surface, was observed to have a tan and black residue on food residue and was not clean to sight and touch. The ice machine was emptied and the deflector plate and ice bin were cleaned and sanitized during this inspection. The floor surface inside of the reach-in refrigeration unit has dried food residue and spillage and in need of cleaning. The Ladies room does not have a self-closing door.

Banko’s, 39 Allen St., West Nanticoke, June 29. Regular inspection; out of compliance. Violations: The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed rodent bait not placed in a bait station. Bait was discarded. An open employee’s beverage container was observed in food prep area. Beverage was removed. Observed cases of food stored directly on the floor in an outside freezer unit, rather than 6 inches off of the floor as required. Person in charge moved items off the floor. Temperature measuring device for ensuring proper temperature of equipment is not available in multiple refrigeration units. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration Observed rusting shelves in multiple refrigeration units. Observed the interior top surface of 1 microwave in the kitchen to be damaged and no longer easily cleanable. Observed deeply scored and stained cutting board not resurfaced or discarded as required. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing glass washer in the bar area was 0 ppm, and not 50-100 ppm as required. Sanitizer container was replaced and appropriate levels were reached. Can opener blades were observed to have a buildup of food residue. Can openers were cleaned. Observed a buildup of yellow residue on the coleslaw cutter stainless steel bowl. Bowl will be cleaned prior to use. Observed a buildup of grease on the hoods above the cooking area. Hoods are scheduled to be cleaned. Observed 3 large flies in the kitchen area.
