No one at the facility with working knowledge of food safety was able to answer food safety questions. Non-food contact surfaces are not cleaned at a frequency to preclude accumulation of dirt and soil of bane marie cover and refrigerated handles. the food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Common bowls were observed stored directly in bulk foods with no handle above the product. Food utensils in the cook line (rice) area observed stored in a container of water which is not maintained at 135°F. Fish items are served raw, however, a written consumer advisory (on the menu, table tent, or placard) is not provided to the consumer on special menus available at counter. Old food residue was observed in the handwash sink, indicating uses other than handwashing. The chlorine concentration in the sanitizing solution of the three-bay warewash sink was 0 ppm, rather than 50-100 ppm as required. Detergent was connected to both detergent and sanitizer pumps on the machine. Machine was cleared, primed and corrected by the inspector during this inspection. Front inside panel of the hood ventilation system above the cook line observed with build-up of static dust in need of more frequent cleaning. Food facility was unable to provide documentation for procedures, monitoring of critical control points, and corrective actions in the HACCP plan submitted to and approved by the Department.
