The following are recent Luzerne County restaurant inspection reports from the Pennsylvania Department of Agriculture. According the agency: “any inspection is a ‘snapshot’ of the day and time of the inspection. An inspection conducted on any given day may not be representative of the overall, long-term cleanliness of an establishment. New Great Wall II, 19 N. River St., Plains, May 15. Regular inspection; in compliance. Violations: Observed a buildup of rust on the metal storage shelves in back area. Interior lids of both chest freezers are cracked and repaired with duct tape. Observed a buildup of food residue on the no-food contact areas of the slicer. Observed a buildup of old food residue along the edges and around the hinges of the bain marie. Observed a buildup of residues along the edges of the shelves in multiple refrigeration units. Observed an accumulation of food debris, grease and webbing under and behind equipment.

Violations; Pre-packaged food (hoagies, muffins) not labeled properly with the ingredient statement indicating any allergen ingredients. Observed a buildup of dust on the fan guard in one reach in cooler. Observed a buildup of old food residue around the nozzle on the creamer dispenser. Observed a buildup of residue on the bottom shelf in one reach in cooler. Person in charge cleaned area during inspection. Three-bay sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Person in charge cut hose during inspection. Employee restroom does not have a self-closing door.

Violations: Three quarts of chocolate milk was offered for sale past expired sell by date of 05/13/2017, two half gallons of 2% milk was offered for retail sale past expired sell by date of 05/14/2017.Expired milk was removed from point of sale during this inspection. The restroom does not have a self-closing door.
Lighthouse, 1019 Shoemaker Ave., West Wyoming. May 16. Regular inspection; in compliance. Violations: Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. The facility will register and pay for someone to take the course that is good for a 5-year term within the next 10 days. The owner will email to this inspector or fax/mail proof of registration and payment within the next 10 days. The facility will obtain a food employee certification within 90 days and email a copy of the certificate to this inspector or fax/mail a copy of the certificate to the Region 3 Field office within 90 days. The inside surface of the soda gun holster, a food contact surface, was observed to have a tan yeast-like residue and was not clean to sight and touch. The soda gun holster was cleaned and sanitized during this inspection. There is a buildup of grease and dust on the hood filter above he fryer. There is a tan yeast-like residue on the inside ceiling, the right wall and wall with door of the walk-in cooler located in the basement.

Violations: Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration Observed a buildup of black residue on the deflector plate in the ice machine. Machine was shut down and will be cleaned and sanitized prior to use. Lights are not shielded or shatter proof over the prep area. Observed filters removed from ventilation system hoods. Filters need to be replaced prior to use. Restroom does not have a self-closing door.
Sam’s Club #8191, 441 Wilkes-Barre Twp. Blvd., Wilkes-Barre, May 16. Regular inspection; in compliance. Violations: Observed a buildup of dust on the fan guard in the meat cooler. Observed A buildup of old food residue around the hinges on the pizza bain marie. Observed a buildup of residue on the surface around the nozzles and behind the ice chute on the self-serve beverage machines. Multiple ware-wash sinks have a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap.
Shanghai Chinese Restaurant, 2450 Memorial Hwy., Ste. 10, Dallas, May 16. Complaint inspection; in compliance. Violation: Soap was not available at the hand-wash sink in the prep are area behind service counter at end of cook line. Soap was placed at sink during this inspection.

Observed a buildup of food residue and dust on the shelves under the liners in the walk-in cooler.

Serpico’s Pizza, 220 E. End Centre, Wilkes-Barre, May 18. Regular inspection; in compliance. Violations: Observed containers with no handles being used as scoops and stored in bulk ingredients. Observed wet wiping cloths in kitchen area, not being stored in sanitizer solution. Interior of plastic prep sink is scored and stained and not easily cleanable. Interior lid surface of chest freezer is deteriorating. Observed a buildup of food residue on the shelf in the bain marie in the front area. Person in charge cleaned shelf at time of inspection. Observed a buildup of dust on the fan guard in the Coca Cola cooler in the front area. Observed a buildup of old soda syrup residue on surface around nozzles on the beverage machine. Observed a buildup of residue on the interior ceiling in the pizza prep bain marie. Employee restroom is not provided with a covered waste receptacle for sanitary napkins. Employee restroom does not have a self-closing door. Observed 50 old rodent-like droppings along the wall and under shelving in the employee restroom.

Violations: The person in charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food employee observed in prep area not wearing a beard cover. A food employee was observed touching a ready-to-eat food with bare hands. Food was discarded. Barrier was provided. Food in multiple refrigeration units stored open with no covering. Foods were covered. Grab-and-go sandwiches not labeled properly with the full ingredient statement including any allergens. Observed cracked shield on the meat tenderizer. Handle on refrigeration unit in cooking area is broken and repaired with tape which is not a cleanable surface. Mechanical ware-washing equipment had a greasy buildup observed inside. No measuring device available to test and ensure that the temperature of the final sanitizing rinse in the mechanical dishwasher is reaching appropriate temperature. Interior of microwave, a food contact surface, was observed to have food residue and was not clean to sight and touch. Microwave was cleaned. Observed old juice residue on the nozzle on the juice machine. Observed a buildup of dust on the fan guard in multiple refrigeration units. Observed a buildup of old syrup residue on surfaces around the nozzles on the soda and juice machines. Observed old food residue on the area above the shaft on the milk shake blender. Observed water pooling on the bottom of the bain marie in the prep area. Food employee observed storing clean food equipment while wet, and not allowing time for draining and/or air-drying. Old food residue observed in the hand-wash sink, indicating uses other than hand washing. Employee restrooms do not have a self-closing door. Observed a buildup of debris in multiple floor drains. Observed food debris in various areas of the facility. Observed water pooling under equipment in the prep area. Observed 10 small flies on equipment in the ware-wash area.
Kingston Food Mart, 101 S. Wyoming Ave., Kingston, May 22. Regular inspection; in compliance. Violations: Four quarts of chocolate milk was offered for sale past expired sell by date of 05/19/17, two quarts of 1% milk was offered for retail past expired sell by date of 005/14/17 and one half gallon of whole milk was offered for retail sale past expired sell by date of 05/19/17. These milk products were pulled from the shelf during this inspection. Observed brown stains inside ware-wash basins. Basins also had jugs and box with plastic bags stored inside. Used cup observed in the hand-wash sink, indicating uses other than hand washing. Cup was removed during this inspection
