Luzerne County Restaurant Inspections: March 8 to 14

The following are recent Luzerne County restaurant inspection reports from the Pennsylvania Department of Agriculture. According to the agency: “any inspection is a ‘snapshot’ of the day and time of the inspection. An inspection conducted on any given day may not be representative of the overall, long-term cleanliness of an establishment. Also, at the time of the inspection, violations are recorded but are often corrected on site prior to the inspector leaving the establishment.” The information is taken from the inspection database atwww.eatsafepa.com. Postal addresses used here are as listed on the state’s website, and may not correspond to the municipalities in which facilities are physically located.

4th Street Pub, 112 N. 4th St., West Hazleton: regular inspection; in compliance. Violation: One bottle of dry vermouth in bar has small winged insect in product. Product disposed.

Bobby O’s Family Restaurant, 300 Main St., Dupont: regular inspection; in compliance. Violations: Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Machine sanitizer was primed and appropriate residual level was detected. Mechanical warewashing equipment had a residual build-up observed along the edges. Observed old food residue on the post can opener blade. Can opener was cleaned. Observed old food residue on cleaned food pans in the storage area. Pans were rewashed. Observed a build-up of food residue along the edge of the Bain Marie. Observed clean food equipment stored wet in a manner that does not allow for draining and/or air drying (wet nesting).

Boyer’s Food Market, 999 W. 15th St., Hazleton: regular inspection; out of compliance. Violations: Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Food employees observed in food prep areas, not wearing proper hair restraints, such as nets, hats, or beard covers. Re-packed bay scallops are not labeled as water added product. Soup commercially processed was only heated to 124°F before being placed in the steam table and not to 135°F for hot holding as required. Assorted time/temperature controlled for safety foods stored covered in walk-in cooler and from previous days operation are in the temperature range between 48°F and 57°F product center. All items were voluntarily disposed due to time/temperature abuse due to improper cooling. Assorted cooked chicken wings held at 65°F to 80°F, in the cold bar display area, rather than 41°F or below as required. Product removed and cooled to 41°F. Assorted time/temperature controlled for safety, ready to eat foods packaged for grab and go are not accurately date marked. Re-packed dip labeled as crab dip is a fish protein (surimi) product. Item to be accurately labeled. Food thermometer is not scaled to measure to +/- 2°F. Bakery hand wash sink has very low water pressure. Bakery hand wash sink has build-up of grime in and around basin indicating use other than hand wash. Fan guard in proofer in bakery has build-up of static dust/mold like build-up.

Crystal Food Barbecue, 44 E. Broad St., Hazleton: follow-up inspection; in compliance. Violations: Observed deeply scored cutting boards not resurfaced or discarded as required. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Proof of employee certification or enrollment in a certification class required within 10 days.

Happy Garden, 32 Village Center, Pheonix St., Duryea: regular inspection; in compliance. Violation: Toilet rooms do not have a self-closing door.

Jimmy’s Quick Lunch, 123 E. Broad St., Hazleton: regular inspection; in compliance.

Lombardo Market Inc., 39 Pittston Ave., Yatesville: follow-up inspection; in compliance.

Martin Mattei Middle School, 120 New St., Pittston: regular inspection; in compliance. Violations: Customer self-service single-use tableware is not displayed in a manner so that only the handles are touched by customers.

Mountain Fresh Inc., 2203 State Rt. 119, Hunlock Creek: regular inspection; in compliance. Violations: Bacon, egg and cheese and sausage egg and cheese sandwiches in hot hold unit at the deli department held at 113°F, rather than 135°F or above as required. It was determined that the sandwiches were placed in the hot hold unit approx. 1 hour earlier. The sandwiches were reheated to 170°F and placed into another hot hold unit. Temperature of the sandwiches in hot hold unit was 160°F 50 min later. The owner stated that they found that if the sandwiches are placed too far forward in the hot hold unit the temperature drops. If kept back little further they hold a temperature greater than 135°F. Rotisserie chicken in the first hot hold unit were maintaining a temperature of 140-145°F. Prosciutto (discard date 2/20/18) and ham (discard date 2/28/18), a refrigerated, ready to eat time temperature control for safety food in the deli case, was date-marked by the facility, but was beyond the 7 day use or sell by date and requires discarding. The items were voluntarily discarded and denatured during this inspection.

Payton’s Place, 5217 Main Rd., Sweet Valley: regular inspection; in compliance. Violation: ice scoops stored with food contact surface in direct contact with counter/soda dispensing machine. The scoops were cleaned and sanitized and placed in containers during this inspection.

Pittston Area Intermediate Center, 120 New St., Pittston: regular inspection; in compliance.

Pittston Beer and Deli Inc./ Chester Chicken, 325 Laurel Rd., Pittston: follow-up inspection; in compliance. Violations: Food employees observed in prep area, not wearing proper hair restraints, such as nets or hats. Observed single serve items (cups and lids) stored in a restroom that has been placed out of order, however toilet and urinal have not been disconnected and appear to be in use. Sliced deli meat, a refrigerated, ready to eat time temperature control for safety food in the Bain Marie area, was date-marked by the facility, but was beyond the 7 day use or sell by date and requires discarding. Foods were discarded. Observed deeply scored cutting boards not resurfaced or discarded as required.

Pizza Hut No. 122212, 179 Susquehanna Blvd., Hazleton: regular inspection; in compliance. Violations: Surface coating on wire racks in walk-in cooler is deteriorated. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Interior surfaces of dish machine have build-up of mineral deposits. HVAC duct directly above dish machine has build-up of static dust / grime. Fan guard in proofer has build-up of static dust.

Polish Amer. Org., 97 Martin St., Wilkes-Barre: regular inspection; in compliance. Violations: Observed rust forming on shelves in the reach in beer cooler. Observed a build-up of dust on the fan in the beer cooler.

Valley Subs Plus, 5355 F Main Rd., Sweet Valley: regular inspection; in compliance.

Banquet Kitchen @ Mohegan Sun, 1280 Hwy. 15, Wilkes-Barre: regular inspection; in compliance. Violations: Observed old food residue on the area around the shaft on the mixer. Observed a build-up of old food residue on the post can opener blade. Can opener was cleaned.

Lino’s Pizza, 653 Wyoming Ave., Kingston: follow-up inspection; in compliance. Violations: One ceiling tile located above the deli slicer is out of the tracking and exposing the area above. There are two other ceiling tiles that are missing/not in tracking. The owner stated the tiles were removed to find a leak in the roof that occurred during a recent snow storm. It was determined that the roof leaked around the base of the heating unit. Snow was removed from the area and the roof has not leaked. The owner stated that the land lord is going to have the roof repaired this spring when the weather breaks. he is hoping that it is repaired by the end of May. The owner has new replacement tiles at the facility but has not gotten them installed.

Mogo @ Mohegan Sun, 1280 Hwy. 315, Wilkes-Barre: regular inspection; in compliance.

Northeast Concession @ Mohegan Sun, 1280 Hwy. 315, Wilkes-Barre: regular inspection; in compliance. Violations: Observed the plastic coating peeling on the wire racks in multiple glass washers. Observed multiple stained and deteriorating citrus fruit slicers. Observed rusting shelves in the walk-in cooler in the south and grove bar areas. Observed a build-up of grease on equipment storage racks in the Clubhouse area. Observed a build-up of residue and dust on the shelves in the Poker bar walk-in cooler. Observed a build-up of food residue on the exterior of the bulk ingredient container in the pizza prep area in Winner’s Circle. Observed a build-up of dust on the fan guards in multiple refrigeration units. Observed temperature gauges on multiple mechanical dishwashers not functioning properly. Observed loose caulking on the interior of the ice machines in the center bar and the south bar. Observed the interior of the microwave in Winner’s Circle to be deteriorating.

P&R Discounts, Gateway Shopping Center Unit 29, Kingston: complaint inspection; in compliance. Violations: Entrance and exit doors located in the front of the facility have gaps with visible light at the floor door junction and does not protect against the entry of insects, rodents, and other animals. Exit door located in the rear of the facility at bottom of stairs, has a gap at the floor door junction and between the doors and does not protect against the entry of insects, rodents, and other animals. Food facility inspection indicates evidence of rodents activity beneath the bread rack near the deli areas, but facility does have a pest control program. There were 10 hard dried rodent droppings observed under the bread rack. The facility is going to sweep up the droppings and monitor for any new activity. If new activity noted will make pest control company aware. An employee stated that they had rodent activity in March to July 2017 but have not had any issues since that time. The employee stated that the pest control company comes in every Monday but she did not have access to the pest control record at the time of this inspection.

Pierce Street Deli, 517 Pierce St., Kingston: follow-up inspection; in compliance. Violation: Baseboard tiles are missing on both sides of the receiving door located in the rear of the kitchen.

Slice’d, 1280 Hwy. 315, Plains Twp.: opening inspection; in compliance.

Super Bounce LLC, 1024 Wyoming Ave., Wyoming; regular inspection; in compliance. Temperature measuring device for ensuring proper temperature of the two refrigeration units at service counter is not available or readily accessible. Pizza was held at 123°F, in the hot hold unit, rather than 135°F or above as required. It was determined that the pizza was in the hot hold unit approx. 50 minutes. The temperature was turned up on the unit. After 30 minutes the pizza was 137°F.

Valley Mart #3, 2999 Bear Creek Blvd., Wilkes-Barre: regular inspection; in compliance.

Wyoming Volunteer Hose Co. #2, 70 Third St., Wyoming: regular inspection; in compliance.

Alta Pizzeria, 380 S. Poplar St., Hazleton: regular inspection; in compliance. Violations: Exterior Bilco door is exhibiting an approximately 1” gap along bottom edge. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Rack in walk-in cooler is rusting.

Arby’s No. 372, 510 Pierce St., Kingston: change of owner inspection; in compliance. Violation: The Quaternary ammonia concentration in the sanitizing solution of the 3-bay ware wash sink was <10 ppm, rather than 200 – 400 ppm as stated on the manufacturers use directions. The empty bag of sanitizer was replaced and the sanitizer concentration was then 200 ppm.

Chuck’s Main St. Diner, 164 Main St., Luzerne: regular inspection; in compliance.

Georgio’s Subs & Suds, 25 N. Laurel St., Hazleton: regular inspection; in compliance. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Thermometers for ensuring proper temperatures of food are not available or readily accessible.

Grand Six Packs To Go, 231 W. Mine St., Hazleton: regular inspection; in compliance. Violations: Ice bagged on premises is not labeled for point of manufacture.

Hunlock Creek Tavern, 7 Old Tavern Rd., Hunlock Creek: regular inspection; in compliance. Violations: Restrooms do not have a self-closing door. Springs are present but disconnected. Commercially processed refrigerated, ready to eat, time/temperature control for safety food (cheese), located in the refrigerator, and held more than 24 hours, is not being marked with the date it was opened. The cheese was date marked during this inspection. Manual ware wash sink at the bar with a build-up of a white residue on the inside surface of the wash compartment.

Valley Pizza & Subs Inc., 732 Route 93, Sybertsville: regular inspection; out of compliance. Violations: No written procedures available for assorted pizzas/stromboli held at room temperature. Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Food employee observed donning single use gloves without a prior hand wash. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Open can of soda is stored on slicing deck of meat slicer. Package of cigarettes is stored on chest freezer by slicer. Food facility has an employee who held a Certified Food Manager certificate; however, the certificate has expired and is no longer valid. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Cutting board on bain marie has build-up of dark residual grime on food contact surfaces. Two chemical sprayers stored underneath service counter are not labeled for content.

Applebees No. 3308, 2 Weis Ln., West Hazleton: regular inspection; out of compliance. Violations: Person(s) in charge are not monitoring establishment operation for compliance with food code requirements. Food employees observed in food prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Cleaned service plates on rack by dish washer are displayed with food contact surfaces up, exposing food contact surfaces to contamination. Floor plate sealant in in use microwaves is deteriorated and not easily cleanable. Top exterior surfaces of dish machine have build-up of grime from back wash. Interior surfaces of dish machine have build-up of mineral deposits inside wash chamber. Several knives stored in storage box in rear kitchen have build-up of old food residuals on blades. Wire racks in walk-in cooler have build-up of mold on shelves and framework. Chemical spray bottle stored inside hood rim above dish machine is not labeled for content. Two chemical spray bottles are stored on ledge inside hood rim above dish machine.

Arcado’s The Next Generation, 47 N. Main St., Pittston: opening inspection; in compliance.

Golden Star, 1019 Main St., Avoca: regular inspection; in compliance. Interior surface of chest freezer is cracked and repaired with duct tape. Observed the interior of the microwave to be deteriorating and in need of replacement. Observed a build-up of grease residue on wire containers holding food in the walk-in cooler. Food was removed from containers. Observed 25 old rodent like droppings under shelves and behind equipment in the kitchen area. Observed a container of unknown white powder in the warewash area not labeled with name of the contents. Container was labeled.

Gumba’s Pizza, 102 Pittston Ave., Pittston: follow-up inspection; in compliance.

Hilton Hotel & Conference Center, 100 Adams Ave., Scranton: regular inspection; out of compliance. Violations: Chicken observed in the cooler area, slimy to the touch/spoiled/not covered or dated. Variety of food in a second cooler area was not stored properly, date or labeled. Seafood in storage containers, in another cooler area, is not labeled with the name of the food or date and sealed properly. Food in two coolers not wrapped, dated or labeled. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in (2) coolers. no paper towels at employee handwash sink in kitchen.

Komenskys, 412 Main St., Duryea: regular inspection; in compliance. Violations: Four severely dented canned items observed in sales area and intended for sale in the food facility. Items were removed from sale. Nut and poppy seed rolls from a licensed bakery are not fully labeled with ingredients including allergens. Observed shelving in the walk-in cooler to be cracked and rusting and no longer easily cleanable. Observed a build-up of dust on the fan guard in one refrigeration unit.

Wendy’s No. 007005, 40 S. Church St., Hazleton: Fan guards in walk-in cooler have build-up of static dust.

Bernardo’s Pizzeria Inc., 612 Main St., Edwardsville: regular inspection; in compliance. Violations: Handle scoop used for dispensing bulk ingredient was lying with the food contact surface directly on the lid of the container. The scoop was cleaned and sanitized during this inspection. Common cup (Souffle cups) used to dispense bulk ingredients. The cups were discarded during this inspection.Common cup (Souffle cups) used to dispense bulk ingredients. The cups were discarded during this inspection. Food facility does not have available proper sanitizer test strips or test kit to determine appropriate Quat sanitizer concentration. Had chlorine test strips and a Quat sanitizer. Ceiling tile above electrical breaker box broken around the conduit exposing the area above.

Friendly Food Mart/Pen Mart, 110 Robert St., Nanticoke: regular inspection; in compliance. Violations: Prepackaged grab and go sandwiches are not labeled to clearly indicate any “Big 8” allergen ingredients and/or the allergen warning statement. Prepackaged grab and go sandwiches are not labeled properly with the ingredient statement. The food facility is not using an approved detergent-sanitizer in both compartments of the two compartment warewash sink. Person in charge directed to discontinue food service until appropriate detergent/sanitizer is provided. Observed a build-up of dust on the edge of the shelves and on the plastic dividers in the walk-in cooler. Working container in back area, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical.

Joe’s Smokehouse MFF2, 163 Bilby Hill Rd., Shickshinny: regular inspection; in compliance.

Litzy’s Lounge, 614 York Ave., Avoca: regular inspection; in compliance.

Lizzas Mezzo Mezzo, 711 N. Main St., Pittston: regular inspection; in compliance. Violations: Observed a build-up of black residue on the interior top edge of the ice machine. Machine was cleaned. Observed build-up of syrup residue around the nozzles on the beverage machine. Observed a build-up of old food residue on the non food contact surfaces on the mixer. Observed a build-up of food residue along the edge and around the hinges on the Bain Marie.
