Malacari’s Produce and Deli, Route 309, Wilkes-Barre, March 30. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Produce room floor has an accumulation of dirt, dust and food particles under tables, equipment, and corners of floor. Lights are not shielded or shatter proof over the produce wrapping machine area. Ceiling tiles missing in the back produce room and customer area, and need to be replaced. Servsafe has expired.
Valley Mart #4, 3230 Route 309, Dallas, March 31. Observed boxed food stored directly on the floor in the walk-in cooler area, rather than 6 inches off of the floor as required. This was corrected during this inspection. No sign or poster posted at the hand-wash sink in the employee restroom area to remind food employees to wash their hands. This was corrected and a sign was posted during this inspection.

Leggio’s Pizza & Italian Restaurant, 1092 Route 315, Wilkes-Barre, March 31. Observed ROP fish thawing in unopened packaging in baine marie. Observed deeply scored cutting board on baine marie not resurfaced or discarded as required. The handwash sink in the food prep area was blocked by pizza bins and wet wiping cloth and not accessible at all times for employee use. Garlic oil was held at 74°F, in the food prep area, rather than 41°F or below as required. Food facility has an original certificate posted, but the location is not conspicuous for public viewing. Walk-in cooler door observed to not close properly.
Lake-Noxen Elementary School, RR 3 Box 270, Harveys Lake, March 31. No violations.
Great Asian Wok 333, 333 West Broad St., Hazleton, March 31. No hair restraints worn by all food prep employees. No thermometers visible in all in-use refrigeration units. Approximately 10 to 15 old dried rodent droppings observed on floor in store room. Ceiling in walk in cooler has build up of static dust. Floor to wall junctions in food prep area have build up of grime.

Oxford Pub – Triple K Saloon, 579 Main St., Edwardsville, April 1. Buildup of static dust and a tan yeast-like substance on walk-in beer cooler ceiling in basement. Observed a buildup of static dust on the fan guard in the refrigerator containing canned beer, located in the bar area.
Ole Tyme Charley’s, 31 South River St., Wilkes-Barre, April 1. Protective light shields are missing end caps over prep area, and dish machine areas.
Martin’s Candy & Cake Supply, Rear 198 Simpson St., Swoyersville, April 1. Observed bags of chocolate wafers stored directly on the floor in sales area, rather than 6 inches off of the floor as required. The handwash sink located in the packaging area does not have hot water available due to broken and removed water heater. Facility will temporarily use hot water from coffee pot until new water heater can be installed. No hot water heater installed to provide hot water to the handwash sink in food packing area. No sign or poster posted at the handwash sink in the packaging area to remind food employees to wash their hands. Soap was not available at the handwash sink in the packaging area.
Golden Star, 1019 Main St., Avoca, April 1. Observed container of chicken stored directly on the floor in walk-in cooler area, rather than 6 inches off of the floor as required. Observed deeply scored cutting boards not resurfaced or discarded as required. Observed cardboard used as shelf liners and to store meat after roast which is not an approved food contact surface.

Logan’s Roadhouse #438, 2310 Wilkes-Barre Twp. Marketplace, Wilkes-Barre, April 2. Mechanical warewashing equipment observed with build up of slimy residue. Observed ice scoop holder with a buildup of dust. Interior of microwave oven, a food contact surface, was observed to have food residue and was not clean to sight and touch. Observed beverage dispensing equipment with an accumulation of syrup residue on non-food contact surfaces. Observed fan vents in the meat room with an accumulation of dust, dirt on non-food contact surface. Paper towel dispenser empty at the handwash sink in the meat room area. Room only used 3-4 days per week and was not in use at time of inspection. Observed a buildup of ice in the walk in freezer.

J & H Beer, 1574 Route 315, Unit A, Plains Township, April 2. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in refrigeration unit. Interior of microwave oven, a food contact surface, was observed to have food residue and was not clean to sight and touch. The food facility does not have the original certificate for the certified food employee posted in public view. PIC will email copy of Serv Safe Certificate to sanitarian.

The handwash sink in the kitchen area was blocked by dishes and not accessible at all times for employee use. Food facility does not have available the correct sanitizer test strips to determine appropriate sanitizer concentration in mechanical warewash machine. Observed clean dishes and equipment stored on shelf alongside handwash sink and subject to splash. Receiving door located in the basement area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Lights are not shielded or shatter proof in the walk in cooler in the basement. Paper towel and soap for handwashing not provided at handwash sink in the kitchen. Webbing observed on ceiling in dry storage area.
